- Who owns and manages the Eye Clinic of Austin?
Dr. Thomas Henderson founded this practice in 1982 and currently is its owner, president and chief executive officer. Administration is lead by the practice manager and partnered with the billing and health information technology supervisor.

- How do I request that my doctor(s) send the Eye Clinic of Austin a copy of my records?
Generally, there are two ways to do this. One is to call your doctor(s) office and find out their exact policy and procedure. The Second, is to go to the Patient Resources page on this website, scroll down to Medical Records Requests and click link to request records be sent to Eye Clinic of Austin. You can fax or mail this document to Eye Clinic of Austin and we can send it to your doctor(s) or for faster service you can send this request form directly to your doctor(s)yourself. Be aware that your doctor's office may charge you a fee for each copy you request as allowed under Federal and State Laws.

- Can I complete or update my registration and medical history forms on-line?
Yes, you can connect to a link for each form by going to the Patient Resources page on this website. You will need Adobe Reader to do this. There is a link to download a free Adobe Reader software. Most of these forms can be completed on line and saved to your computer. To submit you have three options you can print and bring with you, fax or mail them to us if there is enough lead time to your appointment or you can email mail them to the billing supervisor. Check carefully, some forms are for new and some for estblished patients.

- So the Eye Clinic of Austin has new software, what can I do to participate and how does this effect me as a patient?
Please be patient with us as we adapt to our new software over the next few months. We expect to be faster and more efficient with your processing in and appointment making with continuous use of the software.
How does this affect you today and in the next few months?
We will have faster more efficient service, the provision of electronic prescriptions sent directly to your pharmacy and better claims and billing management.
Over the next year we will be building on this system to bring you electronic medical records, which will offer you a more efficient service, including electronic medical history, faster records transfers in emergencies, letters and reporting to your other doctors and even better claims and billing services.
You will be asked to provide and review all your registration information, to let us scan all your insurance cards and drivers license into your account and then to take a picture of you to match you to the record.
This helps us all become a little more green, efficient and secure. We thank you for your cooperation and assistance.

- Can I request a copy of my medical record at Eye Clinic of Austin?
Yes, go to the Patient Resources page of this website. Scroll down the page and you will find links to the release form to request a copy of your record. There is a $25.00 fee for each copy. Once we receive your signed release form and the $25.00 fee we will process your request and release the records to you or your designee within 15 business days.

- Why does Eye Clinic of Austin want your drivers license and to take your photo?
We have new software! We will be asking you to participate by reviewing your registration information, providing all your insurance cards and providing your drivers license and finally we will take your picture.
We will be scanning all of your insurance cards into your account. Be sure to bring your cards with you to each visit and let us know if you have a new card or new insurance coverage.
We will scan your drivers license into your account to for two basic reasons to prove to insurance that we have positively identified you and to have on file for check writing and credit card transactions.
We will take your photo to be appended to your account for chart identification purposes. We will eventually be going paper chartless and your photo helps the doctor and staff identify you and to be sure that they are reviewing the correct patient's medical information.
